Billing and Payment Policy:
2. Appointment Changes: Patients can modify or cancel appointments up to 24 hours before the scheduled time by using the patient assistance form or by calling the office. Late cancellations or missed appointments will incur a $75 fee.
3. Upfront Costs: Patients are responsible for any insurance-related costs, including co-pays, deductibles, and self-pay amounts. These costs will be charged at least 24 hours prior to the appointment.
4. Insurance and Reimbursement: Grace Mental Wellness will assist with insurance claims but cannot guarantee coverage. Patients should understand their insurance benefits and are responsible for any uncovered costs.
5. Adjustments for Future Costs: Billing may be adjusted based on prior Explanation of Benefits (EOB) statements, which could affect charges for future appointments.
6. Payment Deadlines: Patients must settle their statements within 30 days. Accounts that are overdue by 45 days will be sent to collections, and a 3% monthly service fee will be added.
7. Outstanding Balances: Appointments may be canceled for accounts with balances over $50 unless a payment plan is arranged.
